Visceral leishmaniasis remains a major public health challenge despite being both preventable and treatable. Eliminating the disease requires more than medical treatment—it depends on early diagnosis, effective sandfly control, improved access to healthcare, and sustained community awareness. Continued investment in surveillance, research, and public health programmes will be essential to reducing transmission and preventing avoidable deaths, particularly in vulnerable and underserved populations.

Visceral leishmaniasis, commonly known as kala-azar or “black fever,” remains one of the world’s most dangerous parasitic diseases, second only to malaria in terms of mortality among parasitic infections. Despite being preventable and treatable, the disease continues to threaten millions of people living in tropical and subtropical regions, particularly in communities with limited access to healthcare and disease control measures.

The disease is caused by Leishmania parasites and is transmitted to humans through the bite of an infected female sandfly. Unlike mosquitoes, sandflies are much smaller and are most active from dusk until dawn. Once the parasite enters the human body, it attacks the immune system and spreads to vital organs, making the disease potentially fatal if left untreated.

One of the biggest challenges in controlling visceral leishmaniasis is that its early symptoms often resemble those of other common illnesses. Patients typically experience a persistent high fever that lasts for weeks, accompanied by severe fatigue and progressive weight loss. As the infection advances, anaemia develops due to the destruction of blood cells, leaving patients weak and vulnerable to other infections.

A defining characteristic of kala-azar is the enlargement of the spleen and liver. The swollen spleen may become significantly enlarged, causing abdominal discomfort and increasing the risk of complications. The liver can also enlarge as the parasite multiplies within the body’s immune cells. Without timely diagnosis and treatment, the disease weakens the body’s ability to fight infections, often leading to fatal complications.

Visceral leishmaniasis primarily affects some of the world’s poorest communities, where inadequate housing, malnutrition, poor sanitation, and limited healthcare services create favourable conditions for transmission. Rural populations living in tropical and subtropical climates are particularly vulnerable, although cases can also occur in urban settings under certain environmental conditions.

Diagnosis usually involves clinical examination supported by laboratory tests that detect the parasite or identify specific antibodies. Early diagnosis is essential because effective medicines are available, and treatment outcomes are significantly better when therapy begins before severe organ damage develops.

Public health experts emphasise that preventing sandfly bites remains one of the most effective ways to reduce transmission. Sleeping under insecticide-treated bed nets, using insect repellents, wearing protective clothing during evening hours, and improving housing conditions can all help minimise exposure. Community-based vector control programmes that reduce sandfly populations also play a vital role in disease prevention.

Many countries have made significant progress in reducing the burden of kala-azar through improved surveillance, rapid diagnosis, expanded access to treatment, and coordinated vector-control initiatives. Continued investment in healthcare infrastructure, disease monitoring, and public awareness remains essential to sustaining these gains and preventing future outbreaks.

Researchers are also working to develop better diagnostic tools, safer medicines, and more effective vaccines to eliminate visceral leishmaniasis as a public health threat. International collaboration among governments, healthcare organisations, and research institutions has strengthened global efforts to control this neglected tropical disease.

Although visceral leishmaniasis is one of the deadliest parasitic diseases in the world, it is also one of the most preventable and treatable when detected early. Increased awareness, timely medical care, stronger disease surveillance, and effective vector control can save thousands of lives every year and move the global community closer to eliminating kala-azar as a major public health challenge.
