Typhoid Risk Gains Attention Across India as Monsoon Fever Cases Rise
New Delhi, August 29, 2026: India’s monsoon season is bringing a growing health concern beyond mosquito-borne diseases, with typhoid emerging as an important contributor to the country’s current fever burden.

Data from PharmEasy’s Fever Watch indicates that overall monsoon fever risk has increased in 186 of the 209 Indian cities tracked by the platform. The national risk score climbed from 47 during the opening week of the season to 61 by August 12, while the number of cities classified as having a high risk increased from one in mid-July to 10.
Typhoid Shows the Highest Test Positivity
Among four illnesses monitored by the Fever Watch system—typhoid, dengue, malaria and chikungunya—typhoid recorded the highest test-positivity rate.
The available data puts typhoid positivity at 25.5%, meaning roughly one out of every four tests was positive. Dengue recorded 8.5%, while chikungunya and malaria stood at 1.8% and 1.4%, respectively.
The figures do not mean that one in four people across India has typhoid. They represent positivity among tests included in the Fever Watch dataset and should not be interpreted as a national prevalence estimate.
Seven High-Risk Cities Driven by Typhoid
Typhoid is currently contributing to the high-risk classification in seven of the 10 cities placed in the HIGH category by the Fever Watch indicator.
The data highlights how India’s monsoon health picture cannot be explained solely by diseases transmitted by mosquitoes. While rainfall and stagnant water can increase dengue and malaria risks, contaminated food and water create another pathway for infection.
Among cities showing notable changes in fever risk are Karimnagar and Ramagundam in Telangana, Chandrapur in Maharashtra and Jaipur in Rajasthan.
Why the Monsoon Can Increase Typhoid Risk
Typhoid is caused by Salmonella Typhi and is primarily transmitted through contaminated food or water.
Heavy rainfall can create conditions in which sewage and wastewater enter or contaminate drinking-water sources, particularly in areas affected by flooding, waterlogging or inadequate sanitation.
Health authorities have repeatedly emphasized safe drinking water, proper sanitation and hand hygiene as important measures against the infection. The World Health Organization also identifies safe water, sanitation, food hygiene and vaccination as key tools for typhoid prevention.
Doctors See More Fever and Waterborne Infections
Recent reports from Gurgaon also indicate that doctors are seeing more typhoid and prolonged-fever patients following the monsoon.
Physicians have linked the increase to waterlogging, possible sewage contamination and unhygienic food or water. At the same time, doctors caution that typhoid has not replaced dengue or malaria as the dominant confirmed monsoon infection in every location.
This distinction is important because several monsoon illnesses can produce similar symptoms. Fever, weakness and body aches may occur with influenza, dengue, malaria or typhoid, making medical evaluation important when symptoms persist.
Pune Reports Increase in Waterborne Illnesses
Pune is also reporting a rise in waterborne infections during the current monsoon.
According to local health data cited in recent reporting, more than 2,700 acute diarrhoeal disease cases, 43 viral hepatitis cases and 102 typhoid-related cases were recorded over June, July and August. Doctors have also reported an increase in typhoid cases and warned that contaminated food and water can spread several gastrointestinal and waterborne infections.
Experts Warn Against Self-Diagnosis
Doctors are urging people not to assume that every prolonged monsoon fever is typhoid.
Some diagnostic tests can produce false-positive results, and symptoms can overlap with other infections. Appropriate testing and professional medical assessment are therefore important before treatment begins.
Antibiotics should not be taken without medical advice. Unnecessary or inappropriate antibiotic use can contribute to antimicrobial resistance, which can make bacterial infections more difficult to treat.
Safe Water Remains the First Line of Protection
Health experts recommend several basic precautions during the rainy season:
- Drink properly treated, boiled or otherwise reliably safe water.
- Wash hands thoroughly with soap, particularly before eating and after using the toilet.
- Avoid food prepared or stored under unhygienic conditions.
- Wash fruits and vegetables carefully before consumption.
- Take persistent fever seriously and seek medical evaluation.
- Follow vaccination advice where typhoid vaccination is recommended.
The WHO notes that typhoid can cause prolonged fever, fatigue, headache, nausea, abdominal pain and either constipation or diarrhoea. Severe infections can lead to serious complications.
A Broader Monsoon Health Warning
The latest data suggests that India’s monsoon fever season involves multiple threats rather than a single dominant disease.
Mosquito-borne infections remain an important concern, but the rise in typhoid signals the continuing importance of clean water, sanitation and food safety. With the rainy season entering a period when fever activity has historically remained elevated, health experts are encouraging residents to remain alert and avoid dismissing persistent fever as an ordinary seasonal illness.
For communities facing waterlogging or sanitation problems, preventing contamination of drinking-water sources may be particularly important in reducing the risk of further waterborne infections.
